Please list any hoof pathologies or concerns, as well as any history of laminitis, abscessing, etc.

Please list any pre-existing conditions, significant injuries, or anything of note medically.

Daily environment (e.g., turnout duration and terrain, grass or dry lot, stalled (if so, duration), riding environment, etc.)

If your horse is ridden, is your saddle checked and fitted regularly for proper fit? If so, who is your saddle fitter? Have you worked with a bit/bridle fitter? If so, who?

Do you work with a body worker or chiropractor? If so, please indicate who and frequency. If you have appointment notes, they are very helpful to me.
